To use the Editable Word forms, click on one (.docx is incompatible with earlier Word programs), choose 'Open with' your Word program, type in the information, print the form, and mail with donation. You can also choose 'Save file' and go through the same steps.
Download Pledge Form (PDF)
To use the PDF form, click on the link above, download the form (use 'Save File') , and open it in Acrobat Reader. Then print the form, fill it out and mail it. This form must be filled in manually.
